To receive your detailed risk assessment report, please upload both packets back to this site. A one time fee will be charged at time of upload, payable through Paypal or use the trial code. For options, please contact John @ jbutgereit@youthriskassessment.com. In return you will receive a report within seven business days from the time you upload your completed assessment. Please click the Pay Now button below to proceed with checkout or submit the trial code. Once the payment has been made, you'll be prompted with a form to upload your assessment.
