Youth Comprehensive Risk Assessment
Thank you for using the Youth Comprehensive Risk Assessment (YCRA). This assessment utilizes two separate packets for
each youth being assessed. The youth themselves will need to complete a “Youth Packet”, and either a parent or a
helping professional will need to fill out a “Helping Professional Packet”. If this is the youth’s first
assessment, please complete the “Initial Helping Professional Packet” and the “Initial Youth Packet”. If this
is a follow up assessment, please complete both the “Follow up Helping Professional Packet” and the “Follow up Youth Packet” To
ensure an accurate assessment, please complete the appropriate packets thoroughly. Please do not leave any sections blank.
Packet’s can be downloaded directly from the links provided on this page.
Once the youth packet and the helping professional packets are complete, use the upload link to submit the packets for review.

Please download the appropriate packets below to your computer.
After you have completed the packet, return to this site and upload your assessment using the link provided.
In return you will receive a report within seven business days from the time you upload your completed assessment.
